The Teachers Health Trust (THT) is a self-funded, nonprofit health insurance plan serving educators in Clark County, Nevada. Eligibility

  • Open to covered THT subscribers, spouses/domestic partners, and dependents
  • Members must be active on a THT medical plan for at least 12 months before beginning pre-operative requirements
  • Candidates must be age 21+ and meet BMI criteria:
    • BMI ≥ 40, or
    • BMI ≥ 35 with two or more weight-related health conditions (e.g., diabetes, hypertension, sleep apnea, hyperlipidemia, etc.)

Program Requirements

Participants must complete a structured preparation process to ensure readiness and long-term success. This includes:

  • Attending an information session with a BARInet provider
  • A full medical and psychological evaluation
  • Six consecutive months of nutrition education visits
  • Graduation from the Wondr Health program
  • Completion of any required lab or diagnostic tests
  • A pre-operative visit to review the surgical plan

Covered Procedures

The THT bariatric surgery benefit covers primary procedures performed at BARInet facilities, including:

Revision surgeries and treatment for complications from outside procedures are not included.

Costs and Coverage

  • Members are responsible for applicable coinsurance, deductibles, and a one-time $5,000 surgery co-pay
  • This co-pay may be paid using FSA or HSA funds
  • Only services performed by BARInet providers are eligible for coverage

Reimbursement Incentive Program

Members who stay engaged with their post-surgical care may qualify to receive their $5,000 co-pay reimbursed over five years. To remain eligible, members must:

  • Attend regular follow-up visits with their BARInet surgeon
  • Participate in dietary visits and support groups
  • Complete the Wondr Health program
  • Maintain ongoing health plan enrollment
  • Achieve or maintain steady weight-loss progress

Reimbursements are distributed at $1,000 per year (up to $5,000 total) as requirements are met.
